CITRUS COUNTY, Fla. – The Florida Department of Health in Citrus County (DOH-Citrus) will be mailing second dose appointment postcards to individuals who received their first dose of the Moderna COVID-19 Vaccine from DOH-Citrus.
Anyone who received their first dose on Jan. 5, 7 and 9 will be receiving an appointment postcard in the mail with a date, time and location to get their second dose from DOH-Citrus.
Each person who received a vaccine from DOH-Citrus will be mailed a separate appointment postcard with a listed date and time. Individuals who live together and received their first dose on the same day may receive separate postcards with different appointment dates and times.
Please choose one date and time listed on either postcard to receive the second dose together. Please do not arrive earlier than 15 minutes prior to your appointment time.
Each individual must bring the COVID-19 Vaccination Record Card they received after getting their first dose, the postcard, as well as another signed Department of Health COVID-19 Vaccine Screening and Consent, available on www.CitrusCountyHealth.org.
There will be consent forms available on site for those who do not have access to the computer, or you can visit any Citrus County library to print the form at no cost. If you do not bring the COVID-19 Vaccination Record Card, you will not be able to receive your second dose from DOH-Citrus.
If you received your first dose from DOH-Citrus and did not receive a postcard by Jan.29, please call 352-527-0068 and press menu option 9 and then 3. Please wait until Jan. 29 to call in order to avoid overloading the phone systems.
DOH-Citrus is reminding residents that regardless if you’ve completed the series or have yet to receive the COVID-19 vaccine, the Centers for Disease Control and Prevention recommends that people continue to wear masks, wash hands often, stay 6 ft apart and avoid crowds and poorly ventilated spaces to help prevent the spread of COVID-19.
