Federal health officials are raising serious red flags over a burgeoning financial scandal in Maine, warning that the state’s autism treatment program may have become a playground for systemic fraud.

Dr. Mehmet Oz, currently leading the Centers for Medicare & Medicaid Services (CMS), announced that a recent federal investigation has uncovered tens of millions of dollars in improper payments, drawing sharp parallels to a massive racketeering scheme recently prosecuted in Minnesota.

The investigation, spearheaded by the Department of Health and Human Services (HHS), scrutinized Maine’s autism services and found that every single claim reviewed—a staggering 100%—contained significant errors or lacked proper documentation.

According to Dr. Oz, investigators identified at least $45 million in questionable payouts. Perhaps more concerning is the sheer speed of the program’s growth; spending on these services surged by more than 50% in just a four-year window, a spike that federal officials say is often a hallmark of organized exploitation.

The situation in Maine mirrors a high-profile case in Minnesota, where fraudsters utilized fake treatment centers to siphon off taxpayer money intended for vulnerable children.

Dr. Oz pointed out that investigators noticed similar “clustering” in Maine, where numerous treatment providers appeared in small, specific geographic pockets without clear reason. In many cases, the paperwork was so thin that officials couldn’t even verify if the children listed in the claims had actually received a formal autism diagnosis.

CMS leadership has placed the blame squarely on a lack of state-level oversight. Dr. Oz characterized the situation as leaving the “cash register unattended,” noting that Maine had apparently never conducted a comprehensive statewide review of the program’s integrity. The federal government has since issued a formal letter to Maine Governor Janet Mills and the state’s health commissioner, demanding a clear explanation of how the state intends to fix these “red flags” and recover mismanaged funds.

While the investigation is still unfolding, the stakes for the state are high. Dr. Oz warned that if Maine does not provide a satisfactory plan to remediate the fraud and tighten its accounting practices, the federal government reserves the right to cut off Medicaid payments to the program entirely. For now, the focus remains on determining exactly how much of the $45 million can be recovered and ensuring that the services actually reach the families who need them most.
