State investigators have arrested a man accused of filing dozens of bogus insurance applications for luxury vehicles he never actually owned in an effort to pocket hundreds of thousands of dollars in fake refunds.
Adam Otrok was taken into custody on August 6, 2026, following an investigation by the Florida Department of Financial Services Criminal Investigations Division (CID). State officials said Otrok submitted 41 fraudulent auto insurance applications using fake and stolen personal identification details.
According to investigators, Otrok’s scheme relied on timing. After applying for policies on the luxury cars, he issued stop-payment orders on his premium checks drawn from an underfunded bank account. Before the insurer could flag the unpaid balance, Otrok requested full cash refunds on those same premiums.
The state confirmed that while Otrok attempted to steal $206,000 through the operation, he managed to collect $44,704 before authorities caught on.
“Florida has long been the law-and-order state, but we are now making it the anti-fraud state,” Chief Financial Officer Blaise Ingoglia said in a statement. “If you want to commit financial crimes, my investigators will be close behind you. Fraudsters beware—your crimes will not go unpunished in Florida.”
Otrok faces charges of organized scheme to defraud, insurance fraud, grand theft, and the fraudulent use of personal identification. A conviction carries a maximum penalty of up to 10 years behind bars.
The Broward State Attorney’s Office is handling the prosecution. State officials are asking anyone with tips regarding suspected insurance fraud to submit reports through FraudFreeFlorida.com.
